Complete and return the Duplicate Policy/Certificate Request form to us by fax at 888-255-2060, secure email or to our mailing address. It only takes about 2 weeks for you to receive the copy. If you only need a description of your policy benefits, contact Customer Connections at 800-869-0355, Ext. 4279 and we can get that information to you within 5-7 business days.
Change your beneficiaries

To make a change to your beneficiary(ies), please complete and return the Beneficiary Designation form to us by fax at 888-255-2060, secure email or to our mailing address. It only takes about 7-10 business days to process your request. Once the beneficiary designation has been updated on our records, you will receive a letter confirming that the beneficiary change has been completed.
